• Scan the QR Code on this page or download the app from the App Store (for iPhone) or Google Play Store (for Android).

• Accept the Terms & Conditions to enter the app.
• Choose SSM Health as your health organization.
• You will be directed to the SSM Health MyChart home page. To set up your login credentials, click the “Sign Up Now” button.
• If provided, enter the MyChart Activation code given to you by your provider’s staff. If you don’t have an activation code, click “Sign Up Online” and complete all the required fields. You will need to provide the last 4 digits of your Social Security Number and your date of birth.
• Set up your user ID and password:
  • Create a unique MyChart user ID that will serve as your login ID. Choose a secure and memorable name for your user ID.
  • Create a strong MyChart password by entering it in both the Password and Retype Password fields. Remember that you can change your password at any time.
• To receive notifications for new information such as results or messages, provide your email address in the designated fields and click “Sign In.”
